Informed Consent Checklist

This checklist thoroughly breaks down all of the information that may need to be included in your informed consent documents for new clients. Keep in mind that different states/provinces and regulatory boards may have additional requirements, or may not require all of the information in the checklist. Some of the items may only apply to practices working with certain populations.

This particular checklist is applicable to individuals, including children. I have a separate checklist for couples and families.
